About Acacia Hospice Care in NJ
Acacia Hospice Care in NJ encompasses a family centered approach that will personalize our services towards the goals, needs and wishes of both patient and their caregivers.
Our team will work closely with the patient’s physician to establish a plan to meet all medical and pain management needs related to the hospice diagnoses. Our supportive services will provide spiritual and emotional counseling as needed, as well as grief support for the family.

Acacia Hospice receives referrals 24 hours a day, 7 days a week as well as requests for information and a Bridge program visit. These requests come from many sources such as the patient’s physician, family members and friends, hospital case managers, skilled nursing and assisted living facilities.
In most cases, individuals who are eligible for hospice benefits can begin hospice care the same day.
